Welcome. Here is what to expect.

New to us? Here is what happens, what to bring and the forms to fill in, so nothing on the day is a surprise.

Accepting new patients

Your first visit

Plan on about an hour and a half for the first visit. Checkups after that take about an hour.

  1. Come about fifteen minutes early, or fill in the forms below at home and bring them.
  2. We take any X-rays you need, unless your last dentist can send recent ones.
  3. The dentist checks your teeth and gums and talks you through what they see.
  4. Most first visits include a cleaning with one of our hygienists.
  5. Before you leave, you get a written plan with costs for anything we suggest, and time to think about it.

What to bring

  • A photo ID
  • Your dental insurance card, if you have one
  • A list of the medicines you take
  • The name of your last dentist, so we can ask for your records
  • Reading glasses, if you use them for paperwork

Forms

Filling them in at home saves you time at the front desk. Bring them with you; please don't email them to us.

  • New patient registration PDF

Two pages: contact details and insurance.

  • Health history PDF

Bring it to your visit. Please do not email it.

  • Records release from your last dentist · ask at the front desk

We fill this one in with you at the desk.

Good to know

  • We see children from their first tooth. Parents are welcome in the room.
  • Nervous about the dentist? Tell us when you book. We go at your pace and explain each step before we start.
  • If you need to cancel, please call a day ahead so we can offer the time to someone else.

Questions before you come?

Call us at (920) 555-0163; the front desk can answer most of them. Insurance questions: see insurance and payment.
