Insurance and payment

We bill your insurance for you and tell you your share before we start.

Plans change, and every plan covers different things. Call us with your plan's name before your visit and we'll check what we can. What your plan pays is between you and your insurer; we can't promise coverage. Call (920) 555-0163.

We file claims with most other PPO plans too. Out of network, your plan may pay less, and we tell you the difference before we start.

Paying without insurance?

Our in-office plan covers two checkups and cleanings a year, with X-rays, for one yearly fee, and takes a set amount off other work. Ask the front desk for this year's price.

Your right to a Good Faith Estimate

If you don't have insurance, or you aren't using it, you have the right to a Good Faith Estimate: a written estimate of the total expected cost of your care. It covers care that isn't an emergency.

  • Ask us for one when you schedule, or at any time, by phone or at the front desk.
  • If your bill is at least $400 more than your estimate, you can dispute the bill.
  • Keep a copy or a photo of your estimate.
